This book came largely from the sources listed below and from the partial bibliography that follows.

Some interviews lasted minutes, some hours. Some people were interviewed once and some dozens of times. Interviews took place
over the telephone, via e-mail, and face-to-face.

A number of those listed retired as colonels or generals but were interviewed about events that occurred when they were lieutenants
or captains; thus, to avoid confusion, I have omitted all military ranks.
