Drug-induced spasm of sphincter of Oddi (e.g., codeine, morphine, meperidine, methacholine, cholinergics) to levels 2–15 times normal
Partial obstruction plus drug stimulation
Chronic pancreatitis
Acute cholecystitis
Small bowel obstruction
Intestinal infarction
